About Ontrack – Home

OnTrack - HOME in Medford, Oregon is a state licensed and CARF accredited residential addiction and substance use treatment program. Their services are available for pregnant women and mothers with children ages one to five. They have a licensed childcare facility on site so moms can concentrate on their recovery.

The goal of their residential treatment is to establish a safe environment in which patients can learn critical skills. This will help to improve their lives and lessen their substance use symptoms. The program is intended to support clients who have been evaluated at ASAM level 3.5. Staff members are available 24/7.

Individual counseling sessions, monthly treatment team meetings, couples or family counseling and at least 20 hours of weekly group counseling are all provided for each participant. In addition to counseling services residents have access to case management, peer delivered services and training in social skills from direct care staff.

Starting at intake and continuing throughout treatment, transition and discharge planning is a crucial component of the program. Each person’s unique goals will be met by the customized process which is regularly evaluated by the client and their treatment team. Along with receiving the required medical and mental health care to aid in their recovery, clients also have their urine randomly analyzed to track their progress.

Their harm reduction approach emphasizes individual choices regarding medication to improve recovery. They focus on preserving wellness, creating connections, discovering purpose, increasing self efficacy and maintaining a safe living environment.

Most of the time an outpatient program is designed for someone who has completed an inpatient stay and is looking to continue their growth in recovery. Outpatient is not meant to be the starting point, it is commonly referred to as aftercare.

Group therapy is any therapeutic work that happens in a group (not one-on-one). Motivational Interviewing (MI) is a clinical approach to helping people with substance abuse issues and other conditions shift behavior in positive ways. It is more goal-oriented than traditional psychotherapy, as MI counselors directly attempt to get clients to consider making behavioral change (rather than wait for them to come to conclusions themselves). Its primary purpose is to resolve ambivalence and help clients become able to make healthy choices freely.

Rational Behavior Therapy (RBT) is a form of cognitive behavioral therapy meant to be short-term and comprehensive. It was intended to help clients become more self-sufficent and move forward without the need for expensive, ongoing therapy. It includes an emotional self-help method called “rational self-counseling,” the purpose of which is to give clients all the skills needed to handle future emotional issues by themselves, or with significantly less professional help.

Trauma therapy addresses traumatic incidents from a client's past that are likely affecting their present-day experience. Trauma is often one of the primary triggers and potential causes of addiction, and can stem from child sexual abuse, domestic violence, having a parent with a mental illness, losing one or both parents at a young age, teenage or adult sexual assault, or any number of other factors. The purpose of trauma therapy is to allow a patient to process trauma and move through and past it, with the help of trained and compassionate mental health professionals.
