About MHS – Contra Costa ACTiOn Team

MHS - Contra Costa ACTiOn Team provides Assisted Outpatient Treatment (AOT) to individuals who qualify for AB1421/Laura's Law. MHS - Contra Costa ACTiOn Team is located in Concord, California.

The program provides both voluntary and court-ordered services through an Assertive Community Treatment (ACT) model for up to 75 eligible adults in Contra Costa County. All services meet the guidelines described in AB 1421 (Laura’s Law) while respecting the choice, autonomy and dignity of individuals struggling with symptoms of serious mental illness and/or co-occurring substance abuse disorders.

The 5 star reviews are fake. Be warned this place is very sloppy and full of complacent staff you can end up waiting a full hour and they can mark you late for your appointment try to call them they never pick up the phone leave a voicemail they never return the call best thing to do is call VES constantly until they pick up a full week before appointment so they know your coming or else the rescheduling could take another year they will screw you over and they will never be held accountable they are getting payed by the VA they could care less

As a Veteran who has heard all the different stories when it comes to the VA process and examinations I was supremely pleased with how great the staff and process was here. All the providers were so helpful, calm and understanding. I wish I did all my exams here but I have nothing but positive things to say. Keep up the great work.

NP Gracia is phenomenal, extremely knowledgeable and his C&P exams are thorough. Complete God sent, especially from the horror stories you see online regarding exams. Night and day compared to other exams I've encountered.

Private insurance refers to any kind of healthcare coverage that isn't from the state or federal government. This includes individual and family plans offered by an employer or purchased from the Insurance Marketplace. Every plan will have different requirements and out of pocket costs so be sure to get the full details before you start treatment.

Military members, veterans, and eligible dependents have access to specific insurance programs that help them get the care they need. TRICARE and VA insurance can help you access low cost or no cost addiction and mental health treatment. Programs that accept military insurance often have targeted treatment focused on the unique challenges military members, veterans, and their families face.

Medicaid is a state based program that helps lower-income individuals and families pay for healthcare. Medicaid covers addiction treatment so those enrolled can use their coverage to pay for rehab. When a program accepts Medicaid the client often pays very little or nothing out of their own pocket.

Outpatient Programs (OP) are for those seeking mental rehab or drug rehab, but who also stay at home every night. The main difference between outpatient treatment (OP) and intensive outpatient treatment (IOP) lies in the amount of hours the patient spends at the facility. Most of the time an outpatient program is designed for someone who has completed an inpatient stay and is looking to continue their growth in recovery. Outpatient is not meant to be the starting point, it is commonly referred to as aftercare.

Intensive Outpatient Programs (IOP) are for those who want or need a very structured treatment program but who also wish to live at home and continue with certain responsibilities (such as work or school). IOP substance abuse treatment programs vary in duration and intensity, and certain outpatient rehab centers will offer individualized treatment programs.
12-step icon12-Step
12-step programs are addiction recovery models based on Alcoholics Anonymous (AA). A number of substance abuse programs (including some drug and alcohol rehab centers) use the 12 steps as a basis for treatment. Beginning steps involve admitting powerlessness over the addiction and creating a spiritual basis for recovery. Middle steps including making direct amends to those who've been hurt by the addiction, and the final step is to assist others in addiction recovery in the same way. 12-Step offshoots including Narcotics Anonymous (NA), Cocaine Anonymous (CA), Dual Recovery Anonymous (DRA), Sex and Love Addicts Anonymous (SLAA) and Gamblers Anonymous (GA).

Completing a drug or alcohol rehab program shouldn't spell the end of substance abuse treatment. Aftercare involves making a sustainable plan for recovery, including ongoing support. This can include sober living arrangements like halfway houses, career counseling, and setting a patient up with community programs like Alcoholics Anonymous (AA) or Narcotics Anonymous (NA).

Sober Living Houses (SLHs), aka sober homes or halfway houses, are safe, substance-free, supportive living facilities for those recovering from substance abuse. Ideal for those who've just been through inpatient or outpatient treatment, SLHs are supervised environments with rules that support sobriety, such as curfews, shared chores, and therapeutic meetings. Residents are also often trained on life skills and coping skills to make it easier to transition into society. SLHs also provide a strong sense of community that can lead to the kind of deep and lasting connections with other sober individuals that supports a new, healthy lifestyle.

Residential treatment programs are those that offer housing and meals in addition to substance abuse treatment. Rehab facilities that offer residential treatment allow patients to focus solely on recovery, in an environment totally separate from their lives. Some rehab centers specialize in short-term residential treatment (a few days to a week or two), while others solely provide treatment on a long-term basis (several weeks to months). Some offer both, and tailor treatment to the patient's individual requirements.

Treatments

Many of those suffering from addiction also suffer from mental or emotional illnesses like schizophrenia, bipolar disorder, depression, or anxiety disorders. Rehab and other substance abuse facilities treating those with a dual diagnosis or co-occurring disorder administer psychiatric treatment to address the person's mental health issue in addition to drug and alcohol rehabilitation.

Mental health rehabs focus on helping individuals recover from mental illnesses like bipolar disorder, clinical depression, anxiety disorders, schizophrenia, and more. Mental health professionals at these facilities are trained to understand and treat mental health issues, both in individual and group settings.

They are tailored to the specific challenges adult clients may face, including family and work pressures and commitments. From inpatient and residential treatment to various levels of outpatient services, there are many options available. Some facilities also help adults work through co-occurring conditions, like anxiety, that can accompany addiction.

Young Adult Program
Young adulthood can be an exciting, yet difficult, time of transition. Individuals in their late teens to mid-20s face unique stressors related to school, jobs, families, and social circles, which can lead to a rise in substance use. Rehab centers with dedicated young adult programs will include activities and amenities that cater to this age group, with an emphasis on specialized counseling, peer socialization, and ongoing aftercare.

Clinical Services

Dialectical Behavior Therapy (DBT) is a modified form of Cognitive Behavioral Therapy (CBT), a treatment designed to help people understand and ultimately affect the relationship between their thoughts, feelings, and behaviors. DBT is often used for individuals who struggle with self-harm behaviors, such as self-mutilation (cutting) and suicidal thoughts, urges, or attempts. It has been proven clinically effective for those who struggle with out-of-control emotions and mental health illnesses like Borderline Personality Disorder.

Research clearly demonstrates that recovery is far more successful and sustainable when loved ones like family members participate in rehab and substance abuse treatment. Genetic factors may be at play when it comes to drug and alcohol addiction, as well as mental health issues. Family dynamics often play a critical role in addiction triggers, and if properly educated, family members can be a strong source of support when it comes to rehabilitation.

Group therapy is any therapeutic work that happens in a group (not one-on-one). There are a number of different group therapy modalities, including support groups, experiential therapy, psycho-education, and more. Group therapy involves treatment as well as processing interaction between group members.

In individual therapy, a patient meets one-on-one with a trained psychologist or counselor. Therapy is a pivotal part of effective substance abuse treatment, as it often covers root causes of addiction, including challenges faced by the patient in their social, family, and work/school life.

Life skills trainings involve all the skills a person must have in order to function successfully in the world. These include time management, career guidance, money management, and effective communication. Truly successful addiction recovery is based on the ability to not only live substance-free, but to thrive. Life skills teaches the practical necessities of functioning in society, which sets clients up for success in life, and therefore sobriety.

I went in yesterday for an exam of both my knees. I left work to come and get this done. The staff was very well prepared for my visit so it was quick. I really appreciate that since I needed to get back to work. The M.A who was helping and interviewing me was VERY nice. She was very pleasant and didn't make me feel like I was being interrogated. The physician who took care of me was so very helpful and nice. She was thorough with her exam and explained the reason for any questions. She got me an x-ray, to which I asked how long it would be because I was on the clock for work and she made sure it happened quickly. The x-ray tech was also very nice. The entire staff was just a joy to work with. Thank you, ladies.

Dr.Tracy Edison was a thorough and clear examiner she completed all test s asked the right questions and was clean clear to the point while also being companionate. I think she is definitely a asset to VES and veterans in general. Very courteous and well spoken. I had my DBQ on my telephone and she did not miss a box on it. Appointment was completed in a timely manner also. I commend this medical professional for her dedication to care and veterans alike. Thank you Dr.Edison!

This has been my third time at MedEvals of California, Concord for a VA compensation and pension exam. They have shown nothing but true professionalism while serving my needs as a Veteran during this time...professionalism is also about caring, showing compassion and a helpful demeanor toward those in need.This office has the medical competence that is needed in this type of business, the team work is outstanding and the professional behavior is truly refreshing.
