We transferred my father to this facility after and I truly wish we never did. He had valuable time after he left the hospital to rehabilitate and get stronger and this facility did exactly the opposite of what he needed.He wouldn't have regular occupational therapy and when he did it was 30 minutes a day. Simply way lower than what is needed for someone to gain strength in the backup on their feet or be able to transition from a bed to a wheelchair.I normally don't leave reviews but this facility honestly needs to be looked at by the department of health. We move my father to encompass and the care is so much better, within the first week we've seen a drastic improvement in his mobility and strength as well as his attitude.Objectively this facility does as little as possible and it is obvious from the lack of landscape cleanup in the front to the food they serve.If you have a loved one that needs services I strongly suggest you look at another facility.

My better half is recovering here after being released from the hospital. Everyone here is very friendly and empathetic. Gail in housekeeping goes above and beyond to help in anyway she can. She brings a big smile to our faces when we see her. Only negative was not answering the call button in a timely matter a couple of times. I brought that up and was resolved. Physical therapy is awesome! Fred is well appreciated with his knowledge and great personality.

I unexpectedly spent 5 post-hospital weeks here, recovering from a bad spinal infection that went septic, so I had the opportunity to use most of the services offered at Haven. Because the program has so many components, I need to separate them to give useful reviews.Nurses and CNAs:Nurses: 5 stars. Director of Nursing, Lori, Amy, Stephanie, all Fantastic!CNAs: 5 stars. All were caring and compassionate. Linda, is one of the most professional medical workers I have ever metTherapists: 2/5 stars beginning, 5/5 stars by discharge.Therapists I was initially assigned were not in tune with my needs.After meeting with Andy and James, the therapists he assigned were excellent. Dierdre, Caroline and Ashley, well done all!Food Services: initially 1/5 stars; at discharge 4/5 starsIzzy started buying protein supplements. Her helper Ky'ah became my concierge, coming to my room every day to show me the menu for the following day and to make substitutions. Both of these employees are GOLDEN!Cleaning: 5/5 starsGloria was cheerfully in my room every day. When my laundry disappeared, she and Linda tracked it down.Medical:Dr. Jatoi is one of the nicest doctors I have ever known. He coordinated 3 outside doctors to make sure that they were all onboard with my treatment at Haven. But within the organization, the flow of information from the doctors to the patients needs to be improved. I was kept abreast of test results, imaging reports and changes in treatment by Lori, who made a point of keeping me informed. The organization needs to implement informational visits to patients at least once/week.My overall assessment of Haven is excellent, not because there were no issues during my stay, but because ALL of the staff seemed willing to reassess and improve what they were doing. It is rare for an institution to be staffed almost exclusively with employees who want to do things better. And it is the employees who make or break any institution, but especially a patient care facility. There is no limit to Haven's potential for continuous improvement with the people it has on its staff.

My mother arrived at Haven Health on August 9th. She was doing well initially, going to bingo and doing her physical therapy. She was able to walk on her own to the restroom and have lucid phone conversations with me. Starting on August 24th, I observed a drastic decline in her condition. She had become disoriented, had no idea where she was, or what time it was(night time or day time), and had taken several falls. I left a voicemail for the director and never heard back. I talked to her nurse, Edgar, who basically alluded to her just having poor health issues and that she has been disconnecting her oxygen from time to time and that was the reason for her disorientation. I visited her today and ensured that her oxygen was on for 5 hours straight and she still was completely disoriented, pouring her ginger ale on top of her salad and having a bunch of cognitive issues, I don't understand why they aren't taking her to the hospital or doing more. She is clearly exhibiting some sort of issue that has just arisen in their care and they are doing nothing to fix it. It appears as though I'm going to have to pull her out of there and take her to the hospital myself which is just sad that you trust these people to watch over your loved ones and you have to be the one to go in there and rescue them.Update 8/28/24- They have responded and taken steps to get her to the hospital to get checked out. I feel much better now and that I have been heard. The Dr. came and talked to me and was able to explain that the cognitive decline was due to a lack of sleep.

Do not take your loved one here for rehabilitation. They don't provide the PT as surgerons/doctors prescribe. Doctor visits are suppose to be once a week but that is not the case. Pain management is not maintained. This place should not be consider a rehabilitation center.

My mother had a terrible experience there. Constantly had a roommate with severe breathing problems so she couldn t sleep. Pills were delivered when the staff felt ready getting access or help to the commode was very frustrating. She d be left there for a half hour and now they send me a bill, I called back and asked for an itemized bill. I ve called twice no response yet unfortunately I don t know if there s a better place in Tucson there s probably worse.

I had a very good experience this year at the Haven. I had excellent RNs and CNAs! I apologize if I omitted anyone, but, Amy*, Stephanie and Rose were great RNs! CNAs: Maria, Kim, Providence, Tenile, Carman and fav, Dina!! Thanks to housekeeping staff: Gayle * and Ashlynn. My experience with the kitchen, we'll, let's just say that no one really listens. But, thanks to Kyah for trying!! And I can't forget my PTs and OTs, Brenna*, Georgia, Diedra and Amanda! Thanks to all of you!!

It was horrible!!!!! The staff was rude, no one answered the 'call" button. The food was adequate at best and anything else came out of a vending machine....I assume there were caregivers, but none of them introduced themselves to me. The wi-fi did not work, their "special beds" were extremely uncomfortable for me and would not turn them off. I take medication for anxiety which was not given to me making my situation even worse. I left after 8 hours. My children were appalled at my treatment. Two sheriff deputies came by after they called. When the deputies realized that my medication had been withheld, they allowed me to leaveand apologized. AMAZING!!! No one should be treated that in that fashion.

We brought my mother in law here with Parkinson s after a hip fracture. The facility looks nice and accommodating, but they re short staffed and couldn t meet her needs.Doctor s orders were 3 hours long of PT a day. She was allotted 30 minutes, which amounted to about 10 by the time they got her there.She was left in diapers all day with very little attention. After 8 days in the facility we never met with or heard from the in-house doc. She had one quick wash down in that time and very little cleaning. The staff did not keep up with her meds and many times skipped them.The staff is lazy, with the exception of a few hard working, caring, compassionate individuals. The good workers usually came down from one of the upper levels, but the majority that she had on the first floor were terrible. The nurse that was supposed to be attending to her was quite often caught watching a movie on his laptop in the hallway. We even had employees warn us about the others, and were told directly that we would never get the care she needs here.We pulled her from this facility and took her home to take care of her. It is a lot of work, but we can do a much better job that this place. She would never have recovered there.Don t take your family here. I discovered after the fact this was the place my sister came to before she died. She was also neglected.
