Bell Trace isn't what it used to be. Ever since COVID-19 it seems the level of care has really diminished. Apparently it's easier for management to blame staffing levels for everything than it is to address core issues. It is probably time for upper management to take a closer look at what level of care they can safely provide versus what they promise. Communication between managers is terrible in terms of follow up because no one writes anything down then things go undone, incomplete or wrong after numerous conversations. CarDon and Associates should take a hard look at this facility and how it's managed.

I have started this review probably 10xs. It's absolutely been one of the hardest reviews for me to leave as we lost my dear Aunt! She was such a beautiful sole! She would light up any room with her smile or even with her warmest hugs! It's been so hard because of the amount of passion I have for this building and company!We found ourselves needing to move her from one facility to another quickly so she could be closer to family and to the medical team she needed!I reached out and spoke with Sandy , a clinical liaison who absolutely spear headed our move into Bell Trace!She worked so quickly and had an answer , a move in time and a room number for us with in 2 hours! Sandy was an absolute rockstar with how quick she moved! She knew how important it was for our family and made it so easy!In times like this it's super hard on everyone and she and the staff at Bell Trace took away that burden on myself and my family! Instead they allowed us to be family and they did all the hard parts!The Day my sweet Amy got picked up from her previous facility, we knew she was in the right hands!Amy was experiencing a lot of pain and the driver was so sweet, caring and compassionate! She did everything in her power to ensure her drive to Bell Trace was as comfortable as possible! When I arrived, I was shown her room! There was a welcome gift on the table for the family! A list of contact numbers so we had them! The nurse immediately came in and welcomed herself even though Amy had not made it there just yet!When Amy got there she was welcome by the DON and her nurse, plus several aids! They helped get her out of the van! Quickly got her into her bed and tucked in! They allowed Amy time to get comfortable, before doing any assessment!I can remember being in tears as I heard her crying in pain , the nurse that day grabbed my hand and promised me Amy was in good hands! She was so gentle and caring!The administrator and social services came and introduced themselves to my grandma, sister and myself and quickly made us feel like a family!Unfortunately Amy's health did decline as expected! But the staff at Bell Trace were quick to notice any change and ensure Amy got the care she needed!Amy ended up needing to spend a few days in the hospital. While there, as a family we decided it was time to put Amy on hospice and make sure she's absolutely comfortable!I again reached out for help from Sandy their Liason! She quickly took another weight off our shoulders and helped organize everything for Amy to return to Bell Trace on hospice!When Amy returned the nursing staff kept us updated on everything! Between hospice and Bell traces nurses we knew exactly what was going on!On the day before Christmas Eve we got the call we was dreading! It was time for our family to say our final goodbyes!The Bell Trace staff was so caring! They checked in not only on Amy but my family as well! They brought a cart of drinks and snacks , so our family didn't have to leave!Ryan was her nurse that evening! He kept her comfortable! Ensured she was never in pain ! Kept my grandmother who was at bedside as comfortable as possible! He was so professional and sympathetic for us during this hard time!Amy passed away in the early hours of Christmas Eve! She wasn't alone because of Bell Trace! My grandmother stayed all night because she felt welcomed to by the staff!Amy was not alone because of the love she could feel from every single staff member that came into her room!Our family was so blessed to have had our sweet Amy there!!Bell Trace became family!! Thank you for me such amazing people!

My 87-year-old father has lived at Bell Trace for three years. Before signing the contract, we were invited to a meal in the dining room, where we were presented with a seven-page menu. Among the items available were hot entrees such as salmon, ham steak, chicken, and pasta with a protein option. There were several large dinner salads (with protein), as well as a selection of sandwiches. Each day there were two chef s specials. The menu was robust and offered a nutritious variety of food.Now, two years into the pandemic, the menu is down to one sheet. Hot entrees are limited to either a hamburger or a hot dog; there are four cold sandwich entrees. There is one chef s special. This is for both lunch and dinner every day of the week. For adults. As my dad says, this is like an elementary school lunch.Residents are required to spend $350 each month in the dining room or lose that money. They are also facing a rent increase while they are being asked to eat substandard meals.The past two years have been difficult for everyone, but to make money on the backs of our oldest citizens is shameful. Their nutrition and well-being should be of the highest priority. Bell Trace can and should do better.

It has been necessary to shift our food services program multiple times in recent years due to covid precautions, nationwide supply chain issues and widespread staffing concerns. I have lived at Bell Trace for about six years and find the staff, across the board, to be caring, professional and readily available when needed. They have taken excellent care of us during the pandemic and recently upgraded the public areas to good effect. We have options of meals brought to our apartments or taken in the bistro and the wait-staff is great. But there is a but: The food, on a scale of 0 --5 (with 5 being excellent) I could not give a higher rating than 2. I have every expectation that this may improve as the meals are getting food-committee attention right now. Bell Trace gets 4 stars overall from me. And what I did not mention but are important are the sense of community at Bell Trace and the care take of the residents' physical wellbeing.

My mother moved to Bell Trace from Ft. Wayne to be closer to family. We chose Bell Trace because they offered Independent & Assisted Living and had rehab and long term nursing all on the same campus. My mom's favorite things about living at Bell Trace: enjoying great meals while meeting and making new friends in the Bistro, writing group, the beauty salon in the lower level, weekly housekeeping, and the great staff. I loved knowing mom never had to make another move and that someone was there to provide help if she needed it 24-hours a day. [Full Disclosure--My mother lived at Bell Trace from 2012 to 2017. I loved Bell Trace so much I came to work here 3 years after my mother moved in!]

After being at a prior facility and then touring BT, we found that staff, management and residents so very kind, friendly and accommodating.I really like how they take care of our parents and that they truly care about each and everyone in the facility.Staff is amazing! When we unable to see our parents during the height of Covid, staff made sure we could communicate, even if it was through a window on cell phones.I highly recommend Bell Trace!!

My mother stayed at little bell for rehab after a surgery. The therapists were very good to her and helped get her to where she needed to be to go home. The residents who lived there always seemed happy. Seems like a great place to live.

My grandfather moved into belltrace and was still a very healthy individual and after 2 months of poor treatment from the help at bell trace he became ill. That was then they gave him the wrong medication not once or twice but three times. I later found out that the management allows the CNAs to pass out meds. (Illegal or not).. Numerous cases like this are all i hear when i mention bell trace. So do yourself a favor if you wish to keep you elderly family around..avoid this over priced excuse of a retirement center.
